Applying
for the Practical Nursing Certificate Program
Application Process Information Sheet
Applicants selected for
the Practical Nursing Certificate Program will start the nursing class
in the fall semester, August, or the spring semester, January.
When applying to
the Practical Nursing Certificate Program you will need to print one
Cover Sheet for Application, one Application for Admission, and three
Applicant’s Reference Forms. The Cover Sheet for Application and
Application for Admission are to be mailed to the address on the top of
those forms. The three Applicant’s Reference Forms should be left with
the three individuals you are using as references and returned by those
individuals via the U.S. mail to the address at the top of the form.
The reference cannot be accepted if it is returned via the fax machine
or hand carried in by the applicant.
